Hi,
For wall hung if you can take the pipe direct out from the back wall you don't need ledge wall. The reason for most wall mount commode is used with concealed tank is to save space as it need ledge wall for making P trap connection unless the outlet is right behind the commode.
If external wall is 9" thick they need to scrape the wall to fit the P trap bend but if you have made sunken slab then beam will create obstruction. So most cases ledge wall is must.
